Parent or guardian refers to someone authorized to pick up or drop off during activity. A parent/guardian will have full access to the account with an option to select the primary and secondary contact for calls or priority access to the account.
An emergency contact is an individual who is authorized to pick up/drop off or make decisions in the event a parent or guardian is unreachable. This section is for emergency contacts only - please do not add parent/guardian information here.
We are set up to direct bill FSCD respite fees. We will require an uploaded copy of your contract to your account. Upon receipt of the FSCD contract with the correct rates attached, AAFS will begin direct billing.

The medical support profile assists our staff in understanding the scope of support required and the authorization to administer medication safely. These medications may only be administered when directed by a parent or guardian.
